But I would have missed that if I hadn't been tracking. Week to week, the changes felt tiny. But looking at Week 1 next to Week 12? Night and day. That summary page is where you'll have your "holy crap, look how far I've come" moment. Trust the process, even when the week-to-week numbers feel slow.
How to Use This Template
- 1 Print all four pages in landscape and 3-hole punch them for your binder. Write the start date for each 4-week block at the top.
- 2 Fill in the summary page first. Record your Week 1 starting values — weight, waist, energy, etc. You'll come back to this at Week 12.
- 3 Each week, fill in one column on the current tracking page. Use the 1–5 scales for energy, appetite, wellbeing, and severity. Note any side effects and your current dose.
- 4 At the end of each 4-week block, fill in the notes section at the bottom — capture your wins, anything notable, and questions you want to ask your provider.
- 5 At Week 12, complete the summary page. Fill in your current values next to your starting values and calculate the change. Then take a moment with the reflection prompts — you've earned it.
