What's on This Template

Pre-Appointment Checklist

Everything to gather before you go — current medications, medical history, family history, insurance info, and your full weight management history.

Questions to Ask

Eight pre-written questions covering the medication, side effects, safety, and cost — plus four blank lines for your own. No more blanking out mid-appointment.

Conversation Starters

Three ready-to-use opening lines so you don't have to figure out how to bring it up. Frame it around your health — not just the number on the scale.

Doctor's Notes Section

Space to capture what your provider says during the visit — next steps, lab orders, recommendations. Write it down while you're still in the room.

From Brandon's Experience:

I'll be honest — I was nervous about bringing it up with my own provider. Even as a paramedic, even knowing the medicine behind it, there was still that moment of "are they going to judge me?" They didn't. It was a straightforward, professional conversation. But I also went in prepared — I had my medical history organized, I knew what questions I wanted to ask, and I framed it around my health concerns, not just the number on the scale. That preparation made all the difference.

How to Use This Template

  1. 1 Download and print the PDF. It's one landscape page — perfect for printing at home or at a library.
  2. 2 Work through the left column the night before your appointment. Gather your medications list, medical history, and think about what you've already tried.
  3. 3 Review the questions on the right and add your own. Write down your #1 health concern — that's your opening.
  4. 4 Bring it to the appointment. Use the conversation starters if you're not sure how to bring it up. Take notes on what your doctor says.
  5. 5 Keep it. Whether you start a GLP-1 or not, those notes will be valuable for whatever comes next.
