During my first month on a GLP-1, I didn't track anything — I just "remembered" how I felt. When my doctor asked about patterns, I had nothing useful to say. The second month I started writing things down, and it completely changed the conversation. We could actually see that my nausea was worst on injection days and mostly gone by day 3. That kind of detail helped us make better decisions together. You don't have to fill in every single day — just capture the ones that matter.

How to Use This Template

  1. 1 Print in landscape and 3-hole punch it for your binder. Write the month and year at the top.
  2. 2 Fill in the dates starting from the correct day of the week for that month.
  3. 3 Each day, jot down your main side effect(s) in the calendar cell and circle the severity number that matches how you felt.
  4. 4 On rougher days, use the Daily Detail Log on the right to note what triggered the side effect and anything you tried that helped.
  5. 5 Bring it to your next appointment. Your doctor can see trends at a glance — patterns that are easy to miss day-to-day become obvious on the calendar view.
