Once I started tracking my sleep, I saw the pattern immediately: injection night was always my worst sleep. So I moved my injection to morning, started eating dinner earlier on injection day, and the difference was night and day. Literally. How to Use This Template

  1. 1 Print one per week. Fill in the week number and dates at the top. Keep it on your nightstand or in your binder.
  2. 2 Fill it in each morning. Takes 30 seconds — bedtime, wake time, circle your quality, check your disruptors, jot a note if anything stood out.
  3. 3 Log those vivid dreams. They're common on GLP-1s and worth mentioning to your provider. The notes row is perfect for this — even a few words helps.
  4. 4 Look for patterns on Sunday. Fill in the weekly summary. Is injection night always your worst? Does caffeine after 2pm tank your quality? The answers show up fast.
  5. 5 Share with your provider. If sleep disruptions are persistent, bring a few weeks of data to your next appointment. It gives them something concrete to work with.
