Some people love the one-page-per-day journal style. Others want to see everything at a glance without flipping pages. I ended up using both — the weekly grid for the quick data and the journal pages for days that needed more detail. Having both options in the bundle means you can figure out what works for you without buying a second product.

How to Use This Template

  1. 1 Print both pages and staple or clip them together. One set per week.
  2. 2 Fill in the dates and your current dose at the top of page 1.
  3. 3 Each day, fill in your metrics and circle your ratings on page 1. Log meals and notes on page 2.
  4. 4 At the end of the week, write your Win of the Week and scan the grid for trends — you'll see them instantly.
  5. 5 Bring your sheets to appointments. A few weeks of grids gives your provider a clear picture in seconds.
